Comparison Study between Analgesic Efficacy of Bilateral Superficial Cervical Plexus Block and Analgesic Drugs in Thyroidectomy Patients Post Operatively

Abstract
Thyroid surgery causes pain and discomfort to the patients especially in the first few hours postoperatively, and usually we used to give analgesic drugs to relieve this pain. Bilateral superficial cervical plexus block (BSCPB) is a regional anesthesia technique that can provide analgesia to the patient after surgery. Aim of study: The aim of this study to assess the postoperative analgesic efficacy of BSCPB in comparison with the analgesic drugs for thyroid surgery. Patient and method: Sixty Euthyroid patients were scheduled for elective thyroidectomy, they assigned into two groups (30 patients in each). Group 1 whom received the BSCPB at the end of surgical procedure by US guide, called the block group, and Group 2 whom received IV analgesia paracetamol and diclofenac IM injection just half an hour before skin closure, called the IV analgesia group. After that we used the numerical rating scale (NRS-11) to assess pain postoperatively and vital signs; BP and PR had been measured. Results: We noticed that mean of this NRS was significantly lower in block group than that in IV analgesia group in Zero and 1st hours from operation and at 4th and 10th hours postoperatively, no pain from all patients of block group (NRS scored zero) which were significantly lower than mean of NRS in IV analgesia. BP and PR were significantly reduced in block group compared to IV analgesia group. Conclusion: The study demonstrates that the Bilateral superficial cervical plexus block performed after skin closure with 0.25% Bupivacaine is an effective and beneficial method for postoperative analgesia for thyroid surgery patients.
