Background: Hemorrhoids, commonly known as piles, present a significant health concern, impacting individuals worldwide. While global understanding exists, research specific to the Indian subcontinent, particularly in regions like Himachal Pradesh, is limited. This study aims to bridge this gap by assessing awareness and knowledge levels among the Himachal Pradesh public, considering the unique sociocultural milieu of the region. Material and Methods: A cross-sectional survey was conducted in Himachal Pradesh between August and November 2023, targeting adults aged 18 and above. The sample size of 400 participants was determined using a 95% confidence level. A Google Form questionnaire, validated by experts, encompassed socio-demographic details and 20 knowledge-related questions. Data were collected online, analyzed using Epi Info V7, and ethical considerations were maintained. Results: Out of 400 respondents, 59.5% were male, and 40.5% were female, with 35.75% from urban and 64.25% from rural areas. Knowledge levels varied, with 29.25% having very good knowledge, 39% good knowledge, 19.5% fair knowledge, and 12.25% poor knowledge. Participants showed commendable awareness of hemorrhoid basics but demonstrated room for improvement in understanding risk factors and preventive measures. Conclusion: This study reveals a baseline awareness of hemorrhoids among Himachal Pradesh residents, emphasizing the need for targeted health education interventions. The findings, consistent with broader Indian literature, underscore the importance of culturally sensitive healthcare strategies tailored to regional nuances.
Hemorrhoids, colloquially referred to as piles, constitute a prevalent health issue with notable implications for the well-being of individuals. Characterized by the inflammation and swelling of veins in the rectum and anus, hemorrhoids pose a significant health concern that requires attention, particularly within the context of the diverse demographic landscape of Himachal Pradesh. While the global understanding of hemorrhoids is well-documented, there is a paucity of research specific to the Indian subcontinent and, more importantly, to the unique sociocultural milieu of Himachal Pradesh [1-2].
Himachal Pradesh, situated amidst the majestic Himalayas, is home to a distinctive population with its own set of health challenges. The prevalence of hemorrhoids within this region is likely influenced by cultural practices, lifestyle factors, and dietary habits specific to the local populace. However, there is a dearth of studies exploring the awareness and knowledge about hemorrhoids in the Indian context, particularly in the northern state of Himachal Pradesh [3-4].
This research aims to address this gap by conducting a comprehensive assessment of the awareness and knowledge levels among the general public in Himachal Pradesh regarding the risk factors, preventive measures, and treatment options for hemorrhoids. Drawing upon existing literature from Indian studies becomes imperative to contextualize our understanding within the cultural and regional nuances prevalent in the Indian subcontinent.
The relevance of this study is underscored by the need to tailor healthcare strategies to the specificities of the Indian population, considering its diverse cultural practices and regional disparities. By incorporating insights from Indian studies, we aim to provide a nuance perspective on the prevalence and management of hemorrhoids, shedding light on factors unique to the Indian context.
Objectives of the Study
The primary objective is to gauge awareness and knowledge about the risk factors, preventive measures and treatment options for hemorrhoids among general public of Himachal Pradesh.
Research Methodology
Research Approach –Descriptive
Research Design- Cross-sectional survey design
Study area: Himachal Pradesh
Study duration- between August 2023 to November 2023
Study population: The study's target population encompassed all adults aged 18 and above who had been residents of Himachal Pradesh for a minimum of 12 months
Sample size- A robust sample size of 400 adults was determined using a 95% confidence level, an estimated knowledge level of 50% regarding hemorrhoids, a precise 5% absolute error margin, and a conservative 5% non-response rate
Study tool: A Google form questionnaire consisting of questions regarding socio-demography and knowledge regarding hemorrhoids was created. The questionnaire was initially pre-tested on a small number of participants to identify any difficulty in understanding by the respondents
Description of Tool-
Demographic data survey instrument: The demographic form elicited information on participants’ background, age, marital status, religion, employment, education and many more
Questionnaire: The questionnaire contains 20 structured knowledge related questions regarding hemorrhoids. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero. Scoring was done on the basis of marks as >80%(16-20)=very good,60-79%(12-15)=Good,41-59% (8-11)=Fair,<40% (<8)=poor
Validity of tool - by the experts in this field
Data collection- Data was collected under the guidance of supervisors. The Google form questionnaire was circulated among the residents of Himachal Pradesh for responses using online modes like e-mail and social media platforms like Whatsapp groups, Facebook, Instagram and Linkedin till the 400 responses were collected
Data analysis- Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage
Ethical Considerations- Participants confidentiality and anonymity was maintained
The goal of the current study was to assess general public knowledge regarding hemorrhoids through a non-experimental descriptive survey. A total of 400 respondents including 238 (59.5%) Male patients and 162 (40.5%) female patients were participated in the study. Among the total, 143 (35.75%) were from urban area and 257 (64.25%) were from rural area.
In the present study 29.25% (117) participants had very good knowledge (16-20 marks) towards hemorrhoids, 39% (156) had good knowledge (12-15 marks), 19.5% (78) had fair knowledge (8-11 marks) and 12.25% (49) having poor knowledge (<8 marks).

The findings of this study contribute significantly to our understanding of the awareness and knowledge levels regarding hemorrhoids among the general public in Himachal Pradesh. The satisfactory level of awareness among respondents regarding basic aspects of hemorrhoids aligns with findings from other Indian studies [5-6]. This foundational understanding is pivotal for early recognition and intervention, emphasizing a consistent pattern in the Indian context.
The identification of lifestyle factors and specific risk factors in this study mirrors observations in a similar Indian setting [4-7]. The multifactorial nature of hemorrhoids, encompassing genetic predispositions and modifiable lifestyle elements, is a common thread across Indian studies.
The relatively high awareness among participants regarding preventive measures and lifestyle changes aligns with findings in Indian populations [8]. This suggests a shared recognition of the importance of proactive strategies for minimizing the risk of developing hemorrhoids.
The awareness regarding treatment options and the significance of seeking medical attention echoes findings in Indian literature [9-10]. This consistency underscores the universality of knowledge regarding the importance of timely intervention in the Indian context.
The acknowledgment of non-surgical treatment options by a substantial proportion of participants aligns with observations in other Indian studies [8]. This recognition emphasizes the diversity of approaches available for managing hemorrhoids.
The awareness of potential complications associated with untreated or severe hemorrhoids and the recognition of the role of a healthy diet and regular exercise in preventing hemorrhoids is in line with findings from Indian populations [7-10]. This shared understanding emphasizes the importance of preventive measures in the Indian context.
The categorization of knowledge scores, with a significant proportion falling into the categories of very good and good knowledge, resonates with patterns observed in other Indian studies [5-6]. This suggests a baseline level of awareness that is consistent across various Indian demographics.
The study's outcomes have important implications for healthcare strategies in Himachal Pradesh, aligning with the need for culturally sensitive approaches highlighted in other Indian studies [7-8]. Collaborative efforts involving local healthcare providers and community leaders, informed by successful interventions in similar Indian contexts, are essential for designing effective awareness programs.
Emphasizing the unique sociocultural milieu of Himachal Pradesh, as well as drawing from findings in other Indian studies [4], underscores the importance of culturally sensitive interventions. The incorporation of local practices and understanding regional disparities is crucial for the success of awareness campaigns.
Limitations and Future Directions
Acknowledging the limitations of self-reported data, as seen in other Indian studies [3-7], the study opens avenues for future research. Incorporating qualitative methods to explore attitudes and practices, as suggested in Indian literature [9], can provide a more holistic understanding of community perceptions related to hemorrhoids.
In conclusion, the findings of this study, when compared with existing Indian literature, contribute to a comprehensive understanding of the knowledge landscape regarding hemorrhoids in Himachal Pradesh. The consistent patterns observed across studies highlight the universality of certain aspects of awareness while emphasizing the need for region-specific interventions to address unique sociocultural and regional nuances.
